Health slowly decreasing

Hello, I am fairly new to haven and hearth and i've noticed that my health has started to decrease very slowly, I thought that it would sort itself out overtime but I ended up dieing, Now on my new character the exact same thing is happening again and i need a way of fixing it before i die again, I am wearing shoes so it's not thorn in my foot, Please tell me what I need to do.

Every time you take violent damage you lose HHP, which is the second number in your Health bar (and the yellow part of your bar). To recover HHP takes time and requires the use of Gauze bandages or leeches (or both). With leeches, your SHP (first number, red bar of the health) will drain so you need to make sure you are well fed or else you'll be knocked unconscious by the leeches. Leeches however are a fast easy way to recover HHP over time, until you have access to gauze.

Make sure you always have shoes on, too. Clogs are easy to make but can be destroyed by animal attacks since they are technically armor, so I'd suggest making Bunny Slippers as soon as you are able to (dry 2 rabbit skins on a rack) since they won't be destroyed. But if you're barefoot and walking in the wilds you'll get thorns which will cause damage. So always try to have your feet protected, stay well fed to maintain your regeneration, and use leeches.

A swamp near a lake or river with fish is an ideal spot to slowly recover, as you can use the bloated leeches as bait, and you'll constantly be fed, and can cycle new leeches as you need to (I went from 30~ HHP to max in a day or so doing this). Basically the start of the game you need to learn how to avoid the dangerous animals (Bears and Boars will attack you without provocation), and to keep yourself fed. Good luck in the hearthlands.

Are you eating enough? when you're "starving" (the food bar is dark red, turning black) you gradually lose HHP until you die.

thanks alot for both the tips, I am not really eating that much becuase i have no real access to food, I'm just fishing now, what is the best method of getting food fast for a low level player in this game?

A couple of good sources of food for new players are: Hazel Nut and Apple trees, and fishing. Catch any chickens and rabbits that you see, they give you meat. You can also find abandoned plots of land with planted crops (be careful and ensure they are abandoned). If there is someone in your area who hunts, you might find corpses of animals along the river banks, butcher them for free food.

I highly recommend getting the ability to build a boat as soon as possible, this makes finding food much easier.

yeah i'm fishing atm when I said new I didin't mean that new just not so experienced, My settlement atm has like 10 boats and timber houses and tanning tubs etc we're just struggling to find seeds for farming for food.

Seems like you need a snack.
I suggest getting hunting and going for Rabbits, its easy way to get Leather and bone and at the same time
its a great source of food. Rabbits are litteraly everywhere so keep your eyes peeled.

I cook all the meat from the rabbits I hunt but it dosen't seem to be enough. I might stick with stocking up alot of fish.

Increase your exploration and survival first off, exploration to see foragables, and survival to make sure the basic natural resources are not under q10. Dry wild windsown weeds on drying racks to get seeds, if you can't find any abandoned unclaimed seeds or crops to take.

For food early on, fishing is the easiest method, but you need someone to hunt rabbits or chickens to get bone to make hooks, and you need to make sure to keep your fishing pole equipped (it sucks being on a lake and having your hook or line break). Also you can eat the spindly taproots, if you have enough of them for spare string for your fishing line; Stinging Nettle is a string you can forage, but it doesn't work as a fishing line, and you can't eat it raw either.

With fishing, remember you need to fish where you see fish jumping in the water; anywhere else you'll never ever catch any. With lures you can go semi-afk and fish for awhile, with bait (leeches and worms), it's much faster but you can't go afk since you must replace the bait after each catch. But two crates in your boat and some fishing gear, you can keep the food going for you and your friends (it's what I did my first week or so playing, almost nothing but fishing and boating around to find other fish).
