Hello, I'm new and I'm lost. So how do you play multiplayer?
I never see anyone in the world and I see a friend who plays at the same time as me.

The world is gigantic and there are only 400ish people on. you ARE playing multi-player. Also, you should probably be thankful you haven't seen people as they would probably just kill you.

Yes, you can, you just haven't seen anyone else yet - its a big map for the number of people.
No, the spawn points are random, unless...
If your friend sets a hearth secret then you can talk to someone in the starting room, and put in their hearth secret. This will open a stairway (down?) which lets you spawn near your friends hearth fire.

The moment you notice your items missing or objects destroyed, you will know you play multiplayer

The game is only 'multiplayer'.
Probably the purest, most true form of mmo I've ever had the pleasure to play (in terms of freedom of action, no hand holding or spoon feeding here).

Every day that passes without running into somebody is another day you live to see the next sunrise, be thankful.
Because one day... You'll look around at your accumulated wealth and assets and realise that not only have you become successful... You've also become a target.

One day you'll get your wish and meet some people and on that day you'll realise the meaning of the phrase "be careful what you wish for".
