dead on ground (nearly dead)

well im nearly dead on the ground and i cant get back up how do i?

When knocked unconscious, you lay on the ground unable to move for a minute or two. When the stars go away, you should just be able to click and move as normal. Note that it's your HardHP (HHP) that determine if you die or not, which is the yellow HP bar. If that is under the max value, you can heal with a bandage (need first aid, wool, and a loom to make or can usually get from a friendly neighbor) or you can use leeches. Be careful with leeches.
