What is the formula for sword damage?
Why use Unarmed over Melee?
I apologize if the first two formulas are on the wiki, I can't find them.

Why used unarmed over melee? Unarmed has a 30% (again, if I recall correctly) penetration. Meaning... if you hit someone for 100 damage, 30 of it automatically goes to SHP and the rest goes to armor if any is equipped (else goes to SHP, too).

The 3/4 modifier was added solely to the damage (I had someone verify), and it's noted on the Combat Actions page (direct me to any out of date pages which need to be updated and I will)
