It's not literal, You just need to meet and kin people for it to show in your list, If you choose to kin them that is.

You're looking at your kin list. When it says you're alone in the world it means you have no kin. As AAlex said, friend other people and it will no longer say that.

It also depends of the time when you're playing, because there are people from all over the world, with different timezone

They're less likely to stab you if you request kin right away. It means you're not a raider in disguise and it's a little bit easier to trust you. People who skirt around a lot, do a few things, get closer, and never kin are more likely to raise alarm and get themselves stabbed.
