Leeches Quality

I've noticed that as I feed leeches and keep them in a chest to loss bloat, they gain quality.

They have a different quality when they're bloated, vrs unbloated. How can I tell which is the "true" quality, and does it matter?
Do they heal more HHP if they are of better quality? They do more damage while sucking.
