Why am I taking damage?

I just started out playing this, and it's pretty fun. I built myself a little camp, did some stuff, searched some other stuff, but now, for some reason, I'm constantly ticking for 1 damage every few seconds and I can't figure out why. I had some leeches earlier that I pulled off, could that have something to do with it?

You might have a thorn in the foot from walking in the forest without shoes on. Clogs break easily. You could also be starving. Leeches will not damage you unless they are currently equipped to your body.

Protip, you can use leeches as shoes.
