If you can get leeches apply them to yourself very carefully and your soft hit points will go down but your hard hit points have a chance of going up. You can do this without the first aid skill which is pretty much useless unless you own a sheep farm to get bandage material.

To get leeches you need to find a marsh which appears lightly bluey-green on the mini-map. There is one that is almost the same colour but which appears white on the minimap and has no leeches. If you stand in the right type of marsh they will appear in your equipment since the naturally glue themeselves to you. You can pick them up and put them in your inventory so that you don't lose soft hit points faster than you like.

Leeches bloat when they drink your blood. Pick them off at that point. Their quality will go up as they suck your blood to a maximum of your constitution score. They can be saved for later use, as they unbloat if you store them for a little bit. And they can also be used for fishbait.

You can also use gauze made out of wool at a loom. Once you have the gauze rightclick with it on your char and you can treat his head. Wool is kinda rare though nowadays.

Well then walk into a marsh with your equipment screen open. Just let one or two leeches at a time suck on you and constantly watch your hhp. It might take a while this way, butit will work.

Just make sure you are not hungry (yellow or better green food bar) and wait. ^^

21 soft hit points is plenty to start with if you are using only one or two leeches. You have to be real careful if you are down to ten soft hit points or so.

Soft hit points come back when you are full, so I like to stand between a marsh and a river fishing when I am healing myself. I eat the fish to stay well fed, catch the leeches to heal myself and use the bloated leeches for bait. Fishing and healing are about as slow as each other so it works out well.
