What does my health score mean

My health score is now at 119/119/118

I was under the impression that the first number was soft hit points, the damage I had to take before I would pass out, that the middle number was hard hit points, the damage I had to take before I would die and the last number was my optimal level for both the other two. But since the last number is now the lowest....

What does my health score mean?

I have no modifiers on anything that I know of except my straw hat. I am wearing plain linen and leather, and no necklaces or jewelry.
