Why do i only have the ladder option?

when i start the game my only option is to go up the ladder. I have seen screen shots of the cellar door leading to cities.

Id really appreciate an explanation, id love to play this game with someone else.

Lastly, what is the best route to go in this game? it took me forever to figure out that i needed exploration to find string, is there any other crucail points that i need to know.

The cellar has been deleted because of griefers walling in the stones with palisades.
