making the combat system work

i've read the wiki, i've fought animal's, ive been attacked and killed, and i still don;t understand, how to initiate a fight properly of my own choice. and not sure why i fail to kill things "beginners" are supposed to be able to kill. please , i expect flaming of ungodly amounts but i'll take it cause i'm tired of trying to figure it out on my own. also checked the read first, couldn't find it but i'll admit its late, an i might just have been to tired to see it.
