I need ti use the extract in the cave or above it ?

It will show what is below you. So if you want to prospect Layer 1 you have to be above ground.

If you want metal from the cave: Use the extract above-ground, it will show the location of ore in the cave.

If you use the extract in the cave, you will have to build a minehole within the cave to go a layer down.

Actually according to Jorb rustroot extract range is based on geometric mean of all three qualities, but he's been wrong before.

Look for Heavy Earth. It's often worth exploring caves near villages, a lot of people will abandon heavy earth as soon as they can mine better ores. You really don't need rust root for initial mining. Use your stone axe to dig into the side of cave walls and see what stone it reveals. Keep doing it in different areas until you find a Heavy Earth deposit. Check the ore list.

Did you look at the link barra gave you?

Heavy earth can be put into a smelter and smelted with a chance of it either turning into cast iron or stone.

More clearly, what will happen is, you will use the prospect option of a non-empty jar of rustroot extract. You will then get either just a message, or a message and a red pie slice graphic. If just the message, it will tell you what the tile directly underneath you, on the level under you, will yield. If also the red graphic, it means there is some sort of ore in the direction of the red thing. Follow it for a bit then try to prospect again in hope of being directly on top of the ore to then be told "There is [insert ore name here] directly below."

If the message says heavy earth directly below then yes there is iron ore under you, but it is the one with the least yield (unless horn silver does also indeed yield iron sometimes, in which case horn silver sucks more spheres than heavy earth for iron alone).

When mining unknown walls you have a simple test if you got ore (which you can smelt), or stone:
Make a stockpile out of one of them, in case it is red you found smeltable ore - grey is stone.
