The verification seems to only be done through the one time buy in & not a subscription.
Though we'll need more information on that to be sure of it. (Only have my experience to go on for this question.) If anyone else who has bought a subscription can comment it would be appreciated; as the going idea is that you have to do the one time buy in to become verified.

When are 'refills'?

Is it at the first of the month, or is it based on the first day you play/creation?
Should I be wasting hours until I get to 0, and then I'll have 12 again tomorrow, or will there be no refill?

Yes, I believe so.
Though it may be set to where it will only refill to twelve hours, for an un-verified account, of gametime. At least that is what has been implied. I believe Loftar made a comment on it; I'll go see if I can find it.

No one but the devs will know this but: Do the free hours reset on the 1st of every month or something like 30 days after you started playing?

Well, it's the first in server time... I haven't burned up enough of my time to find out yet, but people could check account pages if they burned up their time this weekend.

OK, so if you pay $15 for your account, you "verify" it. And if you "verify" it, you get 14 hours every week? AND access to Classic Haven???
