The circumstances of the game are what they are. If you can't handle permanent damage due to lag spikes then this might not be the game for you, as -- quite irrespective of what we do with wounds -- you will under any and all circumstances run the risk of losing your char entirely due to lag spikes.
Unavoidable consequence of what we are trying to build here, unfortunately.
The fundamental case for permanent damage rests in the fact that I think that all characters should be possible to "grind down" over time in PvP.
Interesting thought jorb. Have you considered making a subclass of wounds that animals deal that could heal a bit faster?(Especially when treated).
This compounding effect of poultice does quite a number on stats when you treat multiple wounds.
