If you walk out of a cave entrance and you find yourself inside a roundpole fenced in area, would you have the visitor buff active? The way I think I understand the buff, I'm leaning towards "no" ..

No, you have to pass through a gate.

What you CAN do, however, is build a gate right on the cave entrance. Literally on the 2 tiles in front of the cave entrance can fit a roundpole fence gate perfectly.
If you leave the gate open (and it is on claimed area) then yes, anyone exiting the cave shall have visitor.

Please note that if they can leave the claimed area they'll lose visitor, though - so they need to be inside a walled area overall.

You can destroy any hand bashable items even if the claim shield is up. It'll require a vandal scent, but they can avoid that visitor debuff I'm pretty sure.

If the gate is right at the cave entrance and there's no way for them to go outside the claim, they can't because they get visitor as soon as they leave the cave.

For example, imagine a fully enclosed and claimed palisade.
Inside that palisade there's a cliff with a cave entrance and in front of the entrance there's an opened roundpole gate.

Anyone coming from the underground will instantly get visitor when going outside and won't be able to destroy the roundpole gate :D

EDIT:

I'm talking about this:

IF on claimed area and with no 'claim holes' for someone to lose visitor, that is, inside a larger walled area (for example) this will insta-visitor anyone coming from the underground and they won't be able to bash the gate. We changed to a pali gate for aesthetic reasons but it also works with roundpole gates. We tested it, it's safe :D

Can also be cheaply abused to claim caves, in the sense of denying access by having the open gate at the entrance, one free tile for the character to land and a fence with a closed gate to deny further movement.

I'm unclear on how starving to death works.

If I go on vacation with Energy 10,000% and Hunger 150%, my hunger might drop to zero. Do I start dying then? Or is it only when my energy is below 2,000% regardless of hunger?

When energy is under 2000%, you fall into starving, then the more you're online with that, the more damage ticks you get, logging off with it pauses the timer, Hunger % is just for food FEP, nothing to do with dying so don't worry.
