Back in legacy it was 5-6 minutes (One tick), although in Hafen it seemed more like 10-15 to me. (It's been a while since i used one.)

Quality on an item will never change unless you change the item into something else (craft or build with it).

There's actually an entire thread on it with a few speculations. I've not seen anything confirmed on what heals one other than soap or chamomile compress (needs high PER * EXP).
