It's starvation health. Missing a meal permanently lowers it. if it hits zero the animal dies. It also modifies the quality of the animal when slaughtered or bred.

thats weird since i always have food available in the Food Trough and it's range covers all my paddocks
and grass is planted everywhere

more than that, hunger meter NEVER goes below 90%

still.. wellfedness of some animals is now at 90%
how comes?

I had that problem until I stomped all the grass in the paddock. I think grass is bad for haven cows.

Are you sure the radius of the trough covers the field? Are you producing enough food? Is the quality of fodder not higher than your animals Q? Any of those reasons can cause it to take a hit to its bar.

The animals will eat exactly when they hit 90% hunger. If you have any part of your field uncovered, and they take a hunger tick when they're in that part they eat grass instead. It's kind of an AI quirk that I feel the devs could modify if they wanted to, but it's a fairly small problem since it's easy to prep for.

I'm guessing this is what happened personally.
