I've had a lot of those, they started at 6, but have fallen down to 2 and 3's in the last few days.
Do they somewhat heal by themselves?

They don't heal themselves. The only ones that do explicitly state it in their description. Have you been getting the healing roots from the fallen tree? If you can show that they're healing from means other than designed methods, submit a bug report.

Could leeches turn them into leech burns perhaps?

I've read some conflicting messages about leeches working against specific wounds (like blunt trauma) but also that they can help restore Hard Health in general.
