2) Does +4 sledge hammer damage scale with quality? (in w7 it didnt scale)

not actually such precise, like +-10% accuracy is enough

40dmg to what? With what skill? Object dmg when destroying or combat dmg with, say, chop? I'm not quite sure what exactly your question is =)

I'm almost certain that currently, it doesn't. However there's possibility that devs add some kind of stamina drain multiplier determined by sledge quality like with scythe is going to get.
