That death scenario is going to be the case with any airlock if there isn't enough time to close a gate. The goal is to give the pursued a chance to get inside, but not the aggressor.
The layout, more or less, is correct. You really need a small house or something else to change layers so that whoever has aggroed you cannot enter allowing combat relations to drop before passing the visitor gate. (For that matter, once ducked into the house, you could simply hearth out.) As pointed out, without the decay, there should be a wall along the two palisade gates on the right side (just with a large enough area to fit a house, minehole, something as a de-aggro area). I'm not 100% sure the fence gates are necessary, but they're built as a precaution given some past shenanigans and exploits.
