You cannot teleport while knocked out in deep water so you'll have to swim to shallow if you get the opportunity to or drown

In shallow water, you're fine, despite your character's head being underwater.

In deep, you'll start losing stamina as if you were treading water, and you can't hearth home or log out. You'll need to hope you have enough CON and stamina to survive until you wake up, and then try to swim for shore. Otherwise, indeed, you'll drown and die.
