How do you get all of the acheivements on steam?

My main concern is the achievement Omen of Ruin, if the world 16 is supposed to be the test trial of permenant world, then that achievement can't be obtained? As world 16 will be like steam version no? Or will achievement appear when world 17 will be launched next to world 16 even if world 16 continues? Also other achievements that I might have missed out on, oh yea does anyone know when will wounds achieveemnts ever come up on steam as steam is still limiting them to under 100 as it is still learning about this game, which who knows how long it will take?
