The tile doesn't have to be programmed to allow 2 items in each tile. The graphics and the mechanics could be separate. The tile having the flag of water/bridge allowing both land units to treat it as land and boats to treat it as water. Allowing a bridge to have a boat travel under while someone stands on the tile isn't that important. The bridge could even be designed that they prevent boat traffic, more like a land bridge with water channel under it.
Another option I would want is for wagons/carts to be able to traverse shallow river tiles. This would require separate river/lake shallow water and probably would need to be built into the map generator.
Being able to terraform shallow water is another option, to connect lakes that are separated by 2 or 3 land tiles, or to extend land slightly out into a lake.
