Add some kind of disease to the game that can be transmitted by midges, Malaria or yellow fever maybe, that start off as minor stat reduction but progress to serious illness over time.

Maybe if the midges were somewhere around a swamp, they could be infected midges. The other ones shouldn't all have disease.

Hmmm +1 I hear jorb likes wounds maybe hell get a kick out of adding a few diseases if were going that route might as well add a few more. Like disease, from walking around barefoot in animal pens, Balantidiasis from contact with infected swine, Giardiasis (causes Beaver Fever, Giggles), Toxoplasmosis from consuming uncooked lamb, pork or goat, or by contact with cat shit, Whipworm from unwashed fruits ( we had a guy get this on the ship you ever see someone poo spaghetti and turdballs? ) ill never eat spaghetti again.

Add midge dungeons. No way to fight them, just have to walk thru while crying and hope you survive to the end.

Please add a random chance to just kill the hearthling instantly upon contact with a midge swarm for maximum immersion.
