World of Warcraft has similar problem. "Rare" items dropped from almost all dungeon bosses quite consistently from the start. These days "Epic" items are in the same situation and there were "Legendary" items that practically every player was guaranteed to acquire, assuming they put in at least basic effort into the game.
Dwarf Fortress has a nice skill/quality system. The more skilled the worker/source materials are, better chances to get a better product. Skills also make you faster in DF, maybe that is not fit for HnH.
This has kind of bad result though. People would make like 100 swords to get 1 masterpiece, then they would have 99 "excellent" swords. And they would be quite cheap for players to buy as there would be so many of them produced.
