If you have unintended leech burns you deserve the allergy tbh. You can make a tea between when you get a leach on you and first bite.

This isn't about established players, it's an annoyingly punishing mechanic for noobs who hit this without any warning.

I had beesting + leech bite before the mechanic was widely known. I lost 50hhp in 30 seconds to midges since each midge bite procs a reaction. That's a bit harsh for beginners to chew on, especially when it's a mechanic that essentially only targets them.

I thought we were discussing unavoidable mechanics (i.e. midges) and how it is supposedly making everything impossible. Your examples are about how easy it is to make a mistake and get punished for that. I'd argue that the way you learn what to avoid and why is part of the fun and appeal, but nevertheless, it has nothing to do with midges themselves. It is about relatively new allergic reactions and you can get those without midges at all. And I think that's the reason why it's "min 3 factors" for the reaction to kick in and not 2. When the rest of the factors are sort of under your control (awareness is another story), midges represent the wild nature. So you either play relatively safe and don't actively seek for 2nd allergy factor after getting one (so if/when midges get you it's OK) or you can gamble and risk it but then might deal with the consequences.

Midge bites are just as unavoidable as leech burns. Don't stand still when the midges are near. Don't let leeches stay on you if you're in a swamp.

Don't forget the nettles can trigger allergic reactions, too. Seems like there's one more I'm forgetting off hand, but so far my experience is that it's random with an increasing chance the more of these things you get. Some things, namely bees, might increase the chances, as I've had multiple of everything at the same time except bees (and leech burns... only seem to ever get one of those).
Opinions expressed in this statement are the authors alone and in no way reflect on the game development values of the actual developers.
