45% is best case scenario from a 40% grievous weapon, but 40% + 20% murderous rage + 20% max roll from concussion is still 80%.
The people running this skill are also not likely to have access to good alchemy to clear the wounds, nor would they necessarily be without other wounds.
Also, permadeath isn't necessarily bad. But right now it only exists for innocent players and hermits, not for faction PvPers.
I could be wrong, but I think you are missing the point.
Since permadeath is no longer a thing except for innocent players and hermits, one of the game's main appeal, has ceased to be.
I long for a time when death in this game still carried the weight as it used to.
