When mining coal, miners should die randomly, causing of tons caveins which will destroy all cupboards in a large area (maybe even on all levels!). Mining already very easy and fun, about 99% players mining all day. We should change this disbalance and make mining more hard!
Give a man a game engine and he delivers a game. Teach a man how to make a game engine and he never delivers anything.

U forgot to mention giant underground worms implementation, which wide opened jaws will emerge from lower levels right under miner (like sand worm from Duna) and instantly eat him (instant death without any corpse to bury ofc) when he stay on one tile for too long).
And lava! We should have as much lava as possible, cause if miner not turns to ashes at least once per week (when he suddenly struck such lava cavern) he will have so much free time, that even may start complain about mining related diseases implementation on forum (and, yes turns to ashes means... u already know what, right?)). And ofc after lava was released, it will fills entire mine and makes it completely inaccessible.
And don't forget about rapid pickaxe deterioration and the need to often sharpen it).
And decay for mine supports - lower u go, often it happens). WIth cave-ins each time when mine support is disappear ofc.
And u know - there is too much light in underground - miners even don't need torches! I'm pretty sure that it is a bug, yes).
And how about methane explosions? I heared it's a big problem for some mines.
And i think about meteorite impacts sometimes, that caused by miner when he accidentally break some shadow orb inside mine wall (like in Terraria), when dig another tunnel to his beloved coal deposit. Ofc meteorite impact should turn entire surface pclaim of that miner into one big crater (with some good q iron ore in middle of it ^_^).
All this things (and mb some more) should be implemented ASAP, cause peoples may start drop this game if mining will be so easy in future and gold and other metals so cheap. Some petty diseases is not enough for sure. Where some challenge for bored miners afterall , yes?))))

a plague would be nice infection spreads through other people. itll cause a massive genocide in the haven world how fun ;)
