They could become infected knicks and knacks each.
Then they would reduce your stats more than usually, until they are healed.
Like percentual amount .

Or maybe their hhp penalty could start rising.
If you dont clean and treat them you would eventually die of the infection.

Beer won't work. Wine maybe. Spirits for sure.

What we need is epidemics such as the flu and plague that can kill off even the strongest characters.
It would spread like wildfire in the villages.

Random unpreventable death is not a good game mechanics in a permadeath environment. It is not even a good mechanic in a non-death environment.

even thou would be the death of me +1

mabye wound left untreated for longtime become scars and is a perma dent on hp?
