Snuffle and infection.

Snuffle:
Snuffle is a disease you can random get being in shallow water, snow biome, or cave. While player affected by snuffle, it will raise a wound that affected:
Being passive outside will slowly raise wound
Being in shallow water, cave, snow biome will faster raise wound
Being in home, lying in bed, drinking hot tea, or eating sizzing type food decrease wound

Infection:
Infection must be a problem for players who uses gentle policy of devs for wounds. When players dont try heal bloody wounds, wait a time when trauma will self-healed, like Cruel Incision, Deep Cut, Deep Cut, Scrapes & Cuts, Severe Mauling, Unfaced, Wretched Gore its can be held several days while players doing their own thing.
The infection will appear if listed wounds will have no attention in some time.
It will raise over time, depends on how hard wounds is.
It must be alternate way to prevent or cancel it. (using ashes, water, snow, gauze)
Healing a initial wound will start slowly decrease infection wound.

I agree that devs should add more wound type things. Infections (multiple types of them) would be nice.
Eating spoiled food could result in food poisoning and stuff.

Stuff like infections and food poisoning should probably scale with your maximum health.
Even the toughest of warriors could be brought down by stomach runs or gangrene.

We should also be able to shoot heads with catapults. Perhaps even infected heads? So if someone goes nearby or picks it up (s)he might be infected.

Yes, moar reasons of death!
I can add my favorite one - the Lightning Strike.
What can be more cute in permadeath game, than raising your char for like a year and catching a random lightning strike turning him into a pile of ash (yeap, no way to inherit that loser). Of course, you can be also blessed with Lightning's little sister - Forest Fire. If you were scared enough of Lightning to afk your char, you can find him next morning you log in standing in the middle of ash desert - all your base was burned down due to the night storm...

No sarcasm, it should be possible. Permadeath should strike just randomly sometimes, reminding about the cheap price of our silly lives.

More diseases would be fun. Contagious diseases would be a lot of fun.

just few ideas

symptoms

cough=small amount asphyxiation every now and then

stomach pain=fep less, satiations go wack

dizzy= cant sprint and opening for combat bigger

headache= stats are less (like concussion)

fatigue+ can only walk but takes stam (lol)

and then we gotta do some dective work to find out what illness we have lol will make it hard to stop spreading illness cause we wont always know what is transmitting it (some close proximity some from handling food or even a certain unhealthy horse in close proximity)
