Well you have hook in your fishing rod, while you cast the fishing rod to fish, hook accidentally grabs your hat and you throw your hat into the water!

You should have a chance to accidentally hook yourself in the eyeball, and pull out your own eyeball giving you a permanent wound that gives you -50 perception.
