Sore back

When you log out without sleeping in a bed, you should get sore back wound when you log back in, decreasing agility etc.
It would heal relatively quickly, but would give players some more initiative to use beds.

As someone who frequently forgets to sleep, the lack of healing and TW reduction is more than enough.

Sore back from lifting/hauling things? Yes, clearly, because life needs to be more hell.

TW and wounds are the biggest reason to use beds. We don't need artificial shit'ere.
