Might be doubleposting

Any game design reason to lift wheelbarrow/sled over the head to enter building and/or cave?

Or it's just complicated programming gimmick?

IMO would be good QoL improvement

It's a matter of "haven't found an elegant way of coding it into the game," I think. There's an update coming Soon™ that might fix this. You may have seen jorb talking about "object controlled objects" over the last couple of years. No ETA on release, though.

I think it's a matter of the hearthling pushing the wheelbarrow rather than it being pulled behind, so interacting with entrances doesn't work the way it does with carts, etc. Wagons also cannot use such things, and arguably they should... probably not going in a house, but maybe a mine hole or such.
