Not true for underground PvE. Some enemies can suddenly spawn from a tile you just mined out, and boreworms can suddenly pop up in front of you (surrounding you if there's two).

To me, this is the bigger issue. When combat starts, while moving to avoid incoming attacks, you need to unequip whatever is in your hand and drag your sword over to your hand equipment slot to be able to use any of your sword moves. Carefully, because a slight misclick will drop it and the enemy will immediately move over it preventing you from picking it back up. (And if you then happen to get KOed and have to hearth back, you lose it forever.) Shift-click sends the sword back to your belt rather than into your hands, and there's no right-click 'equip' flower menu option either, you can only manually drag it to the equipment window (or, for custom clients, the specially displayed hand slot). I don't care about the exact implementation but I really would like an easier way to equip my sword during combat.
