I like this mainly because of the whole idea of the 'rescue' that would be required and all the thrills, danger and drama that could result from those attempts. D&D 's -10 HP rules are effectively the same thing, and many other games have used the concept of a short 'critical' window in which 'life-hangs-in-the-balance' to add some extra drama and thrill to PvP. I think if animal attacks resulted in this one-hour-window-or-die situation that would be perfectly reasonable middle ground between the old world 3 system which repeatedly inconvenienced you and the new Rage-quit inducing situation. It would encourage people to work together, in a survival situation having someone to treat your injuries really will save your life and that's not something the game really reflects now.
As for PvP I think having the person go into this 'critical' at reaching zero HHP (not the same as KO at zero SHP) would be appropriate but Murder would still give the ability to 'coup de grâce' and kill them permanently effectively ending the critical period immediately.
P.S. Some kind of medical herbal salve being required would be good, this could even be a mini-game, each time the bandages are re-applied one or more randomly selected salves are required necessitating a hasty search for the required herbs or a visit to an apothecary. Higher level characters require higher quality salves.
