I like this. The buff from drugs should provide dimishing returns for a given period of time on a scale (so that use is always cumulative but becomes miniscule over time). Addiction should be something that debuffs and you can use the DOC to compensate for the debuff but it just makes the addiction debuff higher.
You should be able to sustain a very long period of 'buffed' status that has values worth buffing. The progression should be sustainable for a long time and also require at least an IRL week for the debuff to wear off.
Rates & buffs should also be affected by DOC as I mentioned and method of consumption (ingestion vs. inhalation). Inhalation should provide greater benefits but ingestion should provide a better cost:benefit ratio.
As far as the 'pain' factor imho having opiates should reduce combat performance but increase con. It would be used as an after-battle treatment for recovery not an in-battle buff (like...coca or qat might).
