The issue is really the 'target audience' that they want to get... Every publisher 'wants as many customers as possible', so working with the permadeath survival idea is really hard until sometimes soon I guess.
Based on what I see, publishers are like copycats, mimicking each other's idea and ripping them off in the name of profiting... Only on rare occasions you see something really unique or creative come out of the mmo industry.
