Siege mechanics need some serious work for one thing. Laying siege to a city is impossibly time and labor intensive.
The belief slider is still in the game. That thing needs to go.
Activities currently are incredibly beneficial to botters versus players actively playing the game. This creates a bot arms race and really sucks for a lot of us.
Combat is pretty silly. Not only is actually getting a kill nearly impossible, fighting someone who actually wants to fight with you involves who has more buckets of water and playing musical chairs. Whether or not that works for you, that's simply poor implementation.
Hunger needs to be reworked so that it doesn't encourage you to dig holes to raise charisma.
Curiosities are sweet but it's time to move on from them as we've learned what we can from them.
It'd be nice if barter stands worked! Just saying.
We need some way to add value to currency beyond their intrinsic metal value or no one but RPers will ever use them.
Animals are dangerous for the first 20 minutes of every world. Their AI needs reworking and they need to be rebalanced. Also more of them.
Resources and the world are way too static right now. The world needs a means of reabsorbing man-made locations that have gone inactive and regrowing itself over time.
etc. etc. etc.
(Personally I'd love it if we could play the game without walls but I don't know if this is viable)
