Alot of the times were mistakes, as in you cannot see where the map border is and logging in after getting stuck outside crashed it, moving out side by mistake crashed it.
Just lots of mistakes while testing.

Map saves in segments, IIRC, so it is theoretically possible that his part of the world did suffer a rollback. I however haven't experienced a rollback in a long while, so I think it's unlikely.

Well, the corpses of the bears are missing, but Delamore explained this as "corpses do not save". Kind of vague, but it kind of rules out a rollback.
