Need help colecting some data

Some of you know me some of you are too new to do so but this is only somewhat relevant to your willingness to fill out the questionnaire. The test is anonymous all data will only be used in statistical form. Everyone is free to not participate if they so desire but if you do I ask that you give sincere answers.

Mind if I ask what this is for? Don't mind filling it out, and don't really matter either way, but am curious as to what the goal is.
