What does the star icon next to the quality number signify? Isn't it normally a blue circle, at least in Ender's? Is this new, or have I just missed it?

Are stars the new Memories of Pain? I'm guessing this piece of stone was mined or chipped using an axe imbued with a star shard? I found a wolverine hide with a similar star.
