A few changes

atm if PEGI were to rate this game it would surely be given an 18+ rating.
This is not good at all since it wouldn't be a safe environment for those younger audience and a few changes shall need to be made.

1. Remove nudity, the default character skin must wear primitive pants and bras would be much preferred
2. Remove blood from combat
3. Add a chat filter to all ingame functions to remove swears and vulgar language
4. Add a report function to remove those that do swear etc..
5. Animal porn must be removed we simply can't have that

Thats all I could think of any added suggestions would be nice.

I'm assuming this person is laughing more at ratings organizations than at anything else. This is, btw, a good opportunity to point out how much I hate rating organizations and how much they can go fuck themselves. A parent that doesn't give enough of a shit to actually take an active interest in the games their children are playing, as opposed to lazily glancing at the back of a box, basically deserves to have their kids playing the moste vile and corrupt shit out there.

If I were a parent, I would love to play GTA 4 with my kids. 'Son, let's go stick it to the man by killing some cops'. That'll prepare him for the revolution, whenever it comes.
