Committing crimes prevents logging off. I'm not sure if it's intentional or not that swamp fever is a death sentence for not only PVPers, but people who've committed assault simply to defend themselves. It's funny that Jorb was just recently talking about new players getting rage early to protect their drying claim from being destroyed, then they add this that just so happens to kill anyone that defensively buys (and uses) rage.
If you log out sleeping in a bed you still get the healing bonus though.
That "bonus" just increases the rate at which the fever progresses similar to an adder bite. That is to say, it will increase quicker before it eventually begins to heal properly if you sleep in a bed. Even after drinking the juice, mine continued to increase because I was not able to properly log off, which seems to be the best way to deal with the fever. For people without the outlaw debuff, sleeping in the bed speeds up the fever progress and the act of logging off lessens the severity, but for people with the outlaw debuff, sleeping in the bed only speeds up what is an extremely severe wound and they cannot technically log off to "treat" it.
