I've got one, they would clutter the wound screen horrendously. They can tweak the healing speed, or the healing amount per tick to make the current wonkiness go away.

I sure wish I could have Sevenless Nicks & Knacks, but even then I'd still be losing 54 HHP to fucking negligible ant-wounds.

Edit: wouldn't each one only last a few hours anyway?

What is your design goal really in that aspect? Is it just to be as sadistic as possible to your players?

I'm sure you've elaborated before what your plans are, or perhaps not, or perhaps you don't want to ever. In any case, having some idea of the inner workers of your mind would be interesting, or catastrophic.

There's plenty of reasons for this other than sadism. Long term, fighters can gain gargantuan stats by living since world start. If perma hhp loss is in, the only way to get these fighters would be to use them extremely carefully in rare instances. Fighters in day-day fights will accumulate wounds that weaken them over time.

So the pvp dynamic is certainly a gameplay reasoning for perma HHP loss. I don't know what jorbs thoughts/intentions are, but it definitely has a meaningful potential to impact gameplay.

I don't have a problem with slow healing. I don't have any complaint about any wound I can treat, even if it takes time/skill/etc. If I get 50, 100, or even 200 Knicks and Knacks I won't complain because I'll heal them. I like the SHP/HHP/MHP system the way it is. Right now my only complaint is the Wretched Gore, and even back in "legacy haven" leeches and gauze didn't allow you to recover in an hour either, you needed skill/effort/time to heal up from HHP damage.

Even when I heal in MMOs, I think a healer shouldn't be a shallow heal bot. I liked it when "protection from elements", stun/silence, and other effects were done for party survival. I love healing, but just not brainless healing, but tactical healing. Also people shouldn't be able to "cure through" bad play... I've had lots of times where Tanks and DPS are stupid and blame me for wipes.

Perma-death, perma-wounds, and we can't even spawn to an alt to get back to the home we made for ourselves out in the middle of nowhere in a vain attempt to stay alive as long as possible...

I say think of this game as a rogue-like MMO. You're going to die. You can't avoid it. Do what you can to mitigate getting the next character going. Maybe you'll even be the Methuselah of the game who hasn't died in 967 days... but you're going to die.

Except if you want those fighters to be able to, you know, fight, you have to expose them to combat to gain access to their combat arts.

use a second account and party them. Worked for me, works for everyone else.
