There's a spring at the bottom of the river, with new water flowing out.
Fix'd-

IRL this also happens, even at places like a beach, you can have a freshwater spring with seawater on top.

The water is not flowing up, it's simply flowing down in both directions.

I'll never understand why is it so hard to apply some terrain post-processing when generating rivers to ensure this doesn't happen.

With the appropriate amount of post-processing, every terrain feature could look like a penis!
