Here to +1 this thread.

This is still an ongoing bug reproduceable in Default.

At 4k resolution with Interface Scale turned above 1.50x (mine is at 2.60x), the hitboxes on items that are at least 2 squares in size extend to a third square.

This means that:
- 2 square wide items become 3 squares wide.
- 2 square tall items become 3 squares tall.
- 4 square items (like armor) take up a full 9 squares.

Note:
Because the hitboxes are too big, it's impossible to click the items to the right of a wide item or beneath a tall item.

___

This also appears to place stack boxes further from the item (to account for the extra squares taken), making it extremely difficult to click items from stacks that take up multiple tiles (like skins).

___

That explains a lot then.

I don't know how many items are out there that are bigger than 2x2, but I recall - on Ender with 1.2x scale. - having similar issues with pumpkins (3x3), so technically any scaling will produce issues at this point (although less noticeable at lower values).
