I was fishing earlier, and I drowned without realizing it. Swimming was not turned on.

If you were in a boat, someone might have bashed it. Not sure that's implemented but a good theory.

No boat, just fishing in a stream around the middle of a river.

I doubt that. You had probably managed to turn it on somehow.
