First I took everything off my character and swam around in deep water for a bit and then I tried dropping everything to the ground and tried swimming around. Nothing worked.

Edit: forgot to mention I swam around until I was almost out of stamina.

Does the quest say something like "greet Alowishus" first? Have to do that one first, then swim, then go back. I don't mess with the quests much, so don't recall the details on this one. It's the only thing I've seen posted that some people miss, but there have been other complaints on this particular one.

Well it does say greet Gerddred but that's after the swim. The order of the tasks is: Take a naked swim, greet Gerddred, visit a cave, and tell Hrossmet of your explouts. So it's not that they need to be completed in order and none of the other quests I've done with more than one task other than reporting back in has needed to be done in order. On that note the "visit a cave" task is already complete as seen here http://imgur.com/a/9vw48

I'll go greet Gergredd and see if I can complete the swim afterwards. Edit: Ok that worked, though I wouldn't say it's something people miss given the picture I linked and the fact there's no indication you have to do the greet task first.

You don't have to complete the quest objectives in order. Most likely you still had something equipped, like a leech, or a tool

You must stay in the deep water for few seconds for the quest to validate.

No, I had everything unequipped, I was still naked when I went and completed the greet task and then took a swim thus completing the quest without reopening the equipment menu until I went to put everything back on.

Idk. Either there was user error or you encountered what must be a very rare bug.

You need to swim around for a little bit. Just taking a dip doesn't count. ^^

I'm assuming there's a "randomish" timer involved, then? I ask because I'm going to make a guess that if you touch shallow water, even if you don't quit swimming, it could reset the timer. I could be completely off base here. I'm just trying a random thought. No idea if it's related or not.

Well if it was user error then this could have been my error, when I tried before posting I was close to shallow water and could have touched it without noticing as I was focusing on my stamina bar to make sure I could get out before drowning (this quest was my first try at swimming since I started playing again and I wasn't sure how quickly my stamina would drain). The second time I tried and succeeded I swam further out in a bit of a U shape.
