This is the kind of thing that tips a man over the edge.
I just finished building this minehole but now it won't complete. A little late on the warning!
“We still, alas, cannot forestall it-
This dreadful ailment's heavy toll;
The spleen is what the English call it,
We call it simply, Russian soul.”
An idea to consider: Tedium, a Feature.

Seems a bit silly. Should block on try to place.

In case the warning would be shown on placement you could map stuff below.
But you have a point, so here is my suggestion to change minehole mechanic:
- unless there is a claim below the minehole should build
- if the area is occupied no ladder is created
- without a ladder you can only go down, you'll be placed on the first free space from the centre but the search for that tile is unable to cross walls
- without a ladder a check is made if the area is free each time someone goes down, should it be the ladder is created
