Taking off all equipment including hand-equipped items, backpack and cosmetic hat and then entering deep water does not complete this part of a quest.

You have to swim around for a little bit. Just barely entering the water does not suffice. :)

Can confirm, you have to siwm a few tiles.

Yeah, not swimming far enough was my problem.

Didn't realize this had already been asked; I figured it was new in world 10 as I had never seen it before.

It seems to trigger on the second tile you swim into for the most part, but lags and such might mean it takes longer. Could be a bit of a random factor in there, too.
